General Objectives for the Ambulatory Focused Advanced Practice Experience

(Corresponding Professional Outcome Ability Goals are Listed in Parentheses)

At the conclusion of the ambulatory focused advanced practice experience the student should be able to:

  1. Demonstrate an understanding of the pathophysiology and treatment of chronic diseases encountered during the experience and/or assigned by the instructor. (1)
  2. Monitor the patient profile, the medical record and other available patient data to assist in drug-related problem identification and resolution. (1,4)
  3. Contribute to the patient data base by taking medication histories, detecting and reporting adverse drug reactions, and encouraging the use of appropriate drug monitoring parameters. (1,4,6)
  4. Develop, implement, and monitor a pharmacotherapeutic patient care plan for each patient assigned identifying:
    1. the pertinent therapeutic drug-related problems
    2. the priority ranking of those problems within the patient
    3. the plan for the drug-related treatment of those problems and the intervention strategies
    4. the indices necessary to monitor the progress of the problem resolution. (1,2,4,6)
  5. Utilize therapeutic monitoring techniques to identify and evaluate achievement of desired therapeutic outcomes. (1,6)
  6. Individualize patient dosage regimens using appropriate monitoring parameters and pharmacokinetics when serum drug levels are available. (1,4,6)
  7. Demonstrate effective patient communication techniques through direct communication, group learning sessions, and/or newsletters. (1,2,5)
  8. Assess patient medication compliance and strategies for improving compliance. (1,2)
