Goal 
The primary goal of this 4-week rotation is to provide the student with experience in the evaluation and
management of adverse drug events. 

Learning Objectives
After completing this rotation the student will be able to:
A. Demonstrate knowledge and awareness of the responsibilities of institutional pharmacists in the reporting
of adverse events.
B. Demonstrate skills in obtaining pertinent background when collecting data about an adverse event.
C. Demonstrate the ability to review and critically evaluate primary literature.
D. Demonstrate the ability to incorporate drug information, patient-specific factors, and other pertinent
information in formulating clear and rational drug therapy recommendations.
E. Demonstrate the ability to communicate with health-care providers in a clear and effective manner both
verbally and in writing.
F. Understand the policies and procedures for evaluating and reporting adverse drug reactions.
G. Demonstrate the appropriate documentation of adverse drug reactions within the institution.
H. Demonstrate knowledge and awareness of the responsibilities of the pharmaceutical industry in the
reporting of adverse events.


Student Responsibilities and Requirements for Successful Completion of the Rotation

A. The student will present one journal article critique and participate in scheduled Clarian Drug Information
Service Journal Clubs.
B. The student will review an Internet site relating to adverse drug events or medication errors.
C. The student will present an educational program to the drug information clerkship students about adverse
events.
D. The student will participate in the completion of one quarterly adverse event report.
E. The student will supervise the monitoring of the Adverse Drug Event Hotline during the final 2 weeks of
the rotation.
F. The student will prepare a brief article for the Clarian Health Partners nursing newsletter. 
G. The student will complete at least one MedWatch submission.
H. The student will complete an end-of-block examination.
I. Other duties as assigned.

Rotation Evaluation Process

A. The student will receive informal, verbal feedback about his or her performance throughout the rotation. 
B. A formal mid-point evaluation will be completed by the student and preceptor.
C. A formal evaluation between the student and preceptor will be completed on the last day of the rotation
and areas that the resident should concentrate on during subsequent rotations will be addressed.
D. All assignments should be completed by the end of the rotation.
