Description:

This advanced clerkship rotation will introduce the student to long term pharmacy practice through Consultant interaction and in-house operations.  The student will develop skills in federal and state regulations that pertain to long term care pharmacy.  The student will have the chance to make recommendations on the monitoring and use of medications.

Goals:

To use knowledge of pharmacokinetics and pharmacology to make recommendations to prescribers.

To improve and enhance communication skills with health care providers.

To gain an understanding of the rules and regulations pertaining to long term care.

To understand how formulary management impacts both pharmacy and customer.

Objectives:

Monitor, assess, and recommend drug therapy for residents of long term care facilities.

Identify the roles of staff in long term care facilities and communicate recommendations as appropriate.

Demonstrate knowledge of state and federal regulations pertaining to medication use and pharmacy services in long term care.

Understand the dispensing procedures of long term care pharmacy.

Demonstrate the ability to educate other health care professionals including nurses, doctors, pharmacists, and other prescribers.

Requirements:

Provide one journal article review or disease state update.

Provide one case presentation to pharmacy staff.

Involvement in one or two small projects to include inservices or drug information.

Perform drug regimen reviews.

Assist with consultant exit review with facility staff at close of consultant visit.

Observe one medication administration pass and provide feedback.

Activities:

Meetings with preceptors to discuss activities and presentations.

Attend monthly consultant meeting.

Review residents’ charts for data to prepare case presentation.

Observe medication administration by QMAs or nursing staff at facilities.

Present medication in-services to health care professionals.

Attend quality assurance committee meetings as they occur.

Perform audits or inspections at nursing facilities.

Attend behavior management meetings as they occur throughout the rotation.

Provide pharmacokinetic consults as needed during the rotation, which may include Aminoglycoside monitoring.

Learn and apply OBRA guidelines as they apply to long term care.

Research drug specific questions that may occur during the rotation.
