Description: A four week advanced practice rotation in a long-term care facility with a consultant pharmacist. Student activities: 1. Assist in the filling and checking of Doc-U-Dose packaging for long-term care facilities 2. Present six Wellness Programs to area Assisted Living facilities 3. Write a Wellness Program on topic of choice 4. Prepare and present a nursing inservice 5. Accompany the consultant pharmacist to long term care facilities 6. Perform medication cart audits at facilities 7. Assist in chart review and recommendations to nursing staff and/or physicians 8. Perform one medication pass audit 9. Compile a comparison and contrast of available automated dispensing systems 10. Review and help implement a venous thromboembolism disease state management program for long term care facilities 11.Compile a comparison and contrast of currently available blood glucose testing machines for future diabetic clinic use 12. Assist the consultant pharmacist with any clinical research and problem solving that might arise. |
