Description:  The clerkship rotation with Cornerstone Pharmacy Services, LLC is a four week advanced practice rotations that provides the student with an overview of the role of the pharmacist in long term care consulting.  During the rotation each student will be expected to assist their designated preceptor in the drug regimen review (DRR) for a variety of patients in various long term care settings.  Responsibilities include chart review and preparation of physician consult notes, where appropriate, to ensure optimization of pharmacotherapy in the geriatric population.  Special emphasis will be given to screening for potentially inappropriate medication use in the elderly while being mindful that multiple and chronic disease states usually exist in this population.  Additionally, students will be involved in the preparation and presentation of in-service programs to support the ongoing educational needs of healthcare professionals within each facility.  The ultimate goal of this rotation is to train students in the provision of pharmaceutical care to manage drug therapy and improve the quality of life of geriatric patients and other individuals residing in various long term care settings.
