Long Term Care/Consulting/Geriatrics
Preceptor: Rhonda Eldridge
Description:
This program will give the student experience in long term care pharmacy
practice. The student will develop skills in
evaluating and monitoring long term care residents.
Student Activities:
1. Weekly meetings with preceptors to discuss activities and/or presentations.
2. Review residents' charts for data to prepare pharmaceutical care plans and
presentations.
3. Prepare written recommendations for management and monitoring of residents.
4. Observe and critique medication administration(s) by nurses or QMA's at
facilities.
5. Present medication in-service presentation(s) to nursing staff and/or
pharmacists.
6. Attend quality assurance committee meetings as they occur during the
rotation.
7. Conduct or design a drug usage evaluation study.
8. Learn to apply state and federal laws e.g. OBRA guidelines.
9. Provide pharmacokinetic consults e.g. vancomycin dosing as needed during the
rotation.
10. Attend rounds with physicians and nurses if possible.
11. Conduct a resident interview.
12. Participate in the management of subacute care patients.
13. Participate in behavior management committee meetings.
14. Supervise or perform medication audits at nursing facilities.
Student Assignments:
1. Arrive on time to assigned location.
2. Prepare three pharmacotherapy care plans.
3. Prepare one formal case presentation to pharmacists.
4. Answer at least one written drug information request (assigned by preceptor)
which pertains to resident(s) being
monitored.
5. Read and prepare to discuss journal articles, handouts, etc., as assigned by
preceptor(s).
6. Maintain active clinical patient profiles.
7. Present an in-service to nursing staff.
8. Attend a behavior management meeting.
9. Attend a quality assurance committee meeting.
10. Conduct/observe a medication administration observation.
11. Participate in a drug usage evaluation study.
