Introduction
Marsh Supermarkets, LLC has 72 supermarkets located in Indiana. Marsh Pharmacy’s are located in 35 of these supermarkets. Marsh pharmacy made a commitment four years ago advance the practice of Marsh pharmacy under the direction of Val Berger RPh, manager of clinical services and outcomes. Marsh furthered their commitment by adding a full time pharmacist, Jamie B Vortherms, Pharm.D., RPh, in the Lafayette area to focus on clinical services and outcomes. 

Description
The clinical community rotation at Marsh Pharmacy is a four-week rotation. The student is expected to maintain a very flexible schedule during the four- week rotation. The student’s scheduled hours may change from day to day.

The purpose of this rotation is to facilitate the student’s development of professional judgment, clinical skills, communication skills, creativity and competency. Throughout the rotation, emphasis will be placed on using the student’s knowledge base and communication skills to improve the quality of care that Marsh pharmacists provide for our patients. 

Assessment of the student’s progress will be assessed daily on an informal basis. All presentations will be evaluated using the Purdue oral presentation evaluation form and will be discussed the day of the presentation.

Objectives
At the conclusion of this rotation, the Pharm.D. Candidate should be able to:

1. Demonstrate the ability to identify and communicate important counseling information and educational information for new and refill medications.
2. Demonstrate the ability to analyze the relevance of DUR’s and make the appropriate recommendations.
3. Demonstrate the ability to review current literature and apply it to a community setting to increase the quality of care of our patient’s.
4. Demonstrate the ability to create a newsletter for a community pharmacy.
5. Demonstrate the ability to develop patient educational displays.
6. Demonstrate the ability to organize, advertise and implement a program for community pharmacy patients.
7. Demonstrate the ability to identify ways to increase the quality of care of community pharmacy patients.
8. Demonstrate the ability to navigate the internet and critically review sites for validity.
9. Identify disease states of unfamiliarity and review the literature to update your knowledge base, make appropriate recommendations and develop systems to keep you informed.
10. Demonstrate the ability to critically review a journal article.
11. Demonstrate the ability to identify vitamins/OTC’s/herbals of unfamiliarity and develop research skills necessary to update your knowledge.
12. Demonstrate the ability to evaluate the differences between medications in the same drug class.
13. Demonstrate the ability to identify new drugs/new disease state information of unfamiliarity and develop research skills necessary to update your knowledge.
14. Demonstrate the ability to teach patient educational classes.
15. Demonstrate the ability to perform a cholesterol screening.
16. Demonstrate the ability to answer drug information requests.
17. Develop an understanding of the importance of involvement in professional organizations.

Activities
Patient Counseling & DUR Approvals (Daily)
The student will start the day with 2 hours (9:00 AM – 11:00 AM) spent in the pharmacy. The student will be responsible for all patient counseling and DUR approvals. The student should spend this time identifying ways to improve the quality of care our patient’s receive.

Motivation and Education (Daily)
The student will be responsible for posting a daily motivational quote and a daily educational topic in the pharmacy. The student will log the previous topics and file the previous information away on a daily basis.


DUR Round Table Discussions (Daily)

The student will be responsible for obtaining and reviewing the DUR reports on a daily basis. The student will be responsible for calculating the ratio of DUR’s to prescriptions, the percent of DUR’s that are irrelevant, the percent of DUR’s that ideally should be acted upon, and the percent of DUR’s that must be acted upon. The
student will review all the DUR’s and discuss their recommendations with the preceptor and other pharmacy
students. The student will be responsible for completing a final report with all of their findings. A hard copy of the report must be turned in to the preceptor.

AJHP, JAPhA, Drug Topics, Pharmacy Times & US Pharmacist Review (1)
The student will be responsible for reviewing the information in the five journals and identifying any information that pertains to community pharmacy. The student will be responsible for identifying ways to incorporate the new information into our practice setting. A hard copy of your recommendations must be turned in to the preceptor.

Pharmacy Newsletter (1)
The student will be responsible for preparing the monthly Marsh pharmacy newsletter for the Lafayette area. A hard copy must be turned in to the preceptor.

Patient Educational Display (1)
The student will be responsible for preparing a patient educational display for the area outside of the pharmacy. 
The preceptor must approve the topic.

Pharmacy Outreach (1)
The student will be responsible for organizing, advertising and implementing our monthly outreach program. Past outreaches include: cholesterol screening, blood pressure screening, blood glucose screening, brown bag session, health fair, etc. The preceptor must approve the topic. 

Pharmacy Monthly Topic (1)

The student will be responsible for selecting, organizing and implementing our monthly pharmacy topic. Past topics include: Cholesterol medications, Osteoporosis medications, Medication compliance, Calcium interventions, Poison Control, etc. The preceptor must approve the topic.

Final Project (1)

The student will be responsible for completing a final project. The project should be designed to increase the care that our patient’s receive. The preceptor must approve the topic.

Internet Resources (1)

The student will be responsible for developing a list of internet resources that will be useful for them in their future practice. A hard copy of the list must be turned in to the preceptor.

Patient Round Table Discussions (1)

The student will be responsible for selecting a patient with a specified disease state from the pharmacy patient base. The student will be expected to prepare for these discussions by reviewing the current literature and applying it to their patient. The student will also be expected to review the cost effectiveness of the medications, any herbal options available, and prepare a list of web sites for health care professionals and a list of web sites for patients. A hard copy must be turned in to the preceptor.

Journal Review (1)

The student will be responsible for presenting a recent journal article. The article must be related to a patient from the community pharmacy and must be approved by the preceptor. The student will critically review the article and then render an opinion concerning the usefulness of this article to the practicing pharmacist. The student should be prepared to answer questions concerning the article. A copy of the article must be turned in to the preceptor before the presentation. A hard copy of the review must be turned in to the preceptor. 

Vitamins/OTC/Herbal Reviews (2)

The student will be responsible for presenting a review on a vitamin, an OTC class and/or an herbal. The preceptor must approve the topic. A copy of each review must be turned in to the preceptor. 

Drug Class Review (2)

The student will be responsible for selecting a drug class and reviewing the differences in the medications. The preceptor must approve the topic and a hard copy of each review must be turned into the preceptor.

Disease State Update/New Drug Review (1)

The student will be responsible for presenting a review on a disease state update or a new drug related to a patient from the community pharmacy. The preceptor must approve the topic. A copy of the review must be turned in to the preceptor. 

Patient Disease State Classes (?)

The student will be given the opportunity to teach smoking cessation, weight management and disease state classes
to Marsh employees. The number of patients enrolled in the classes will determine the number of classes and topics.

Cholesterol Screenings (?)

The student will be given the opportunity to participate in at least one cholesterol screening after being trained by the preceptor. The procedures and counseling requirements will be discussed prior to the screening. 

Drug Information Requests (?)

The student will be responsible for providing immediate verbal response based on background knowledge or available information resources and/or performing extensive literature searches to drug information requests from all Lafayette Marsh pharmacies. 

Professional Association Meetings (2)
The student will have the opportunity to attend the Tippecanoe County Pharmaceutical Association Meeting and the Central Indiana Pharmacists Association with the preceptor.

Indiana Pharmacist Alliance Board Meeting & Indiana State Board Meeting (1)
The student will have the opportunity to attend the IPA board meeting and Indiana State Board of Pharmacy meeting in Indianapolis.
