AMERICAN PHARMACEUTICAL SERVICES
CONSULTANT PHARMACIST ROTATION
DESCRIPTION: This advanced clerkship rotation will introduce the student to long term care pharmacy practice. The
student will develop skills in evaluating, monitoring and providing medication recommendations for
residents of long term care facilities.
GOALS: To apply student’s knowledge of pharmacology and pharmacokinetics in making
recommendations to other health care providers.
To gain an understanding of the clinical management of geriatric and mentally retarded
patients.
To learn federal and state regulations pertaining to pharmacy services and medication use in
long term care facilities.
To enhance skills in communicating with health care professionals and the public.
OBJECTIVES: Monitor, assess, and recommend pharmacotherapy for long term care residents.
Communicate pharmacotherapy recommendation to appropriate individuals.
Demonstrate knowledge of and apply federal and state regulations pertaining to pharmacy
services and medication use in long term care facilities.
Demonstrate an understanding of long term care pharmacy dispensing procedures.
Demonstrate an ability to interact with and provide information to other health care
professionals.
REQUIREMENTS: Develop and present two (2) drug inservices to nursing staff.
Provide one (1) patient case presentation to staff pharmacist.
Perform a minimum of sixty (60) drug regimen reviews.
Perform one (1) consultant exit review with facility administrator and/or director of nursing.
Perform one (1) drug utilization review based on pharmacy dispensing data.
Observe one (1) medication administration pass with written report of observations.
ACTIVITIES: Weekly meetings with preceptors to discuss activities and/or presentations.
Review residents’ charts for data to prepare case presentation.
Prepare written recommendations for management and monitoring of residents.
Observe and critique medication administration by nurses or QMA’s at facilities.
Present medication in-service program(s) to nursing staff and/or pharmacist.
Conduct or design a drug utilization evaluation study.
Attend quality assurance committee meetings as they occur during the rotation.
Perform medication audits/inspections at nursing facilities.
Participate in behavior management committee meetings as they occur during the rotation.
Learn and apply federal and state laws as they apply to long term care facilities (i.e. OBRA
guidelines).
Provide pharmacokinetic consults as needed during rotation.
Read and prepare to discuss articles and handouts as assigned by preceptors.
