1.
General Statement
The objective will be to focus on
the necessary knowledge and experiences required to safely and accurately work
as a provider of pharmaceutical care in the long term care setting. The student
will receive hands-on experiences in various aspects of the drug distribution,
administrative, and clinical functions of a long term care pharmacist.
2. Goals and Objectives
 | Participate in administrative activities related to long
term care pharmacy management and/or coordination of clinical activities. |
 | The student will be organized, well prepared, and able
to communicate clearly. |
 | Provide
medication therapy monitoring and assessments for individual residents. |
 | Make recommendations concerning drug therapy to the
physician or other appropriate health care professional. |
 | Recognize
residents most at risk for medication-related problems. |

Identify, resolve, and
prevent medication-related problems.
 | Work as part of the resident care team to
plan and provide care. |
 |
Understand how the social and psychological aspects of
aging impact medication therapy. |
 |
Describe the economic and financial aspects of providing
pharmacy services for the elderly. |
3. Clerkship Activities
 | Review drug distribution process, functionality of
computer systems, and automated dispensing systems. |
 |
Observe and participate in the procedures for medication
order, |
acquisition, security, storage and
control, quality assurance, and stat order procedures.
 |
Describe licensure and accreditation standards of the
pharmacy and |
facilities.
 |
Discuss the role of state health inspections for licensure
of facilities and |
the impact on pharmaceutical services.
 | Complete a project pertaining to appropriate use of
medications within the facility, principles of fiscal management within a
health-care system, or other topics as designated by the preceptor. |
 | Make one formal presentation to patients, community
group, medical staff or pharmacy staff on topic assigned by preceptor. |
 | Assist the pharmacist in medication order evaluation and
collect pertinent historical, diagnostic, and laboratory patient data. |
 | Evaluate medical information to identify potential
problems and communicate with prescriber any recommendations for change in
therapy. |
 | Monitor pharmacokinetic dosing of vancomycin and
aminoglycosides as assigned. |
 |
Review/revise existing protocols for monitoring medication
use. |
 |
Communicate therapeutic recommendations to residents,
caregivers, and other health professionals. |
 |
Review and interpret resident information to identify
problems. |
 |
Describe common clinical manifestations of disease and
adverse drug |
events that are unique to older
individuals.
 |
Report and assess adverse events in a manner consistent
with the |
facility’s policies, including use of a
national or regional adverse drug
reaction reporting program.
 |
Price the medications on a monthly basis for the residents
followed. |
 |
Describe the role of payment for medications and for
consultant pharmacy services. |
 | The preceptor will schedule meetings with student to
provide feedback on clinical performance and to review progress on assigned
project(s). |
- Attendance/Late Assignments
·
Failure to turn in projects on time will result in a one letter
grade reduction for that assignment.
·
Tardiness (> 15 minutes late without an excused absence) more than
4 times in the rotation will result in an automatic reduction of one letter in
final grade.
·
The preceptor should be notified as soon as the student realizes
he/she will be tardy or absent.

6. Additional Information
a)
Meals/Lunch: If you bring your lunch, there is a food refrigerator and
break area located in the pharmacy. You may also leave the facility for lunch.
There are several restaurants in the surrounding area.
b)
Sick Days: If you are unable to attend due to illness or emergency,
notify the preceptor as soon as possible by phone. The preceptor reserves the
option to require "make up" days or additional assignments due to days off.
c)
Attire: All students are expected to dress in an appropriate
professional manner. Calculators and student ID badges are required. Lab coats
are optional.
d)
Student Hours: The usual work day for the Pharm.D. student is 8AM - 5
PM. Alternative schedules will require approval by preceptor. The preceptor
reserves the option to change the schedule as required for special projects or
to allow the student to attend in-services.
e)
Location: PCA Pharmacy is located on Constant Comment Place just
minutes off the Blakenbaker exit at I-64.
