Long Term
Care/Geriatrics/Consulting
Description: This rotation is a four week block designed
to provide the Pharm.D. candidate with experience in consultant pharmacy
practice. The student will develop skills in evaluating and monitoring
long-term care residents. The rotation shall emphasize the unique
pharmacologic, pharmacokinetic and therapeutic choices for the geriatric
patient.
Activities:
- orientation to the organization of the long-term care
facility, role of the consultant pharmacist in the institution, including
committee involvement.
- accompany preceptor to nursing facility or other
designated location.
- review residents’ charts for data assimilation to
prepare 5 written care plans
- prepare written recommendations for management and
monitoring of residents.
- observe and critique medication administration by
nursing in the facility
- present inservices for nursing and pharmacy staff
- attend quality assurance, psychotropic review and
other committee meetings
- round with certified ostomy and wound care nurse on
staff
- round with medical director (physician) each week
- supervise or perform medication audits in a facility (CVT)
- weekly meeting with preceptors to discuss activities
or presentations
- read and prepare to discuss articles and handouts as
assigned
- review, informally, four articles from a primary
journal pertinent to geriatric pharmacotherapy
- attend IV class and rounds if possible
