Northside Pharmacy Long-Term-Care Vincennes, IN
Description: An advanced clerkship rotation in the long-term care environment. Students assist the pharmacist with consulting work for three nursing homes serviced by the pharmacy. Students will review patient profiles, evaluate medication regimens for accuracy and completeness, review lab values, allergies, drug interactions etc. Patients mostly encountered during this rotation include geriatric and some mental retardation patients. Many chronic disease states are encountered as well. Student activities: 1. Complete 3 nursing home inservices 2. Attend meetings with preceptor (quarterly review, psychotropic review etc) 3. Serve as the drug information source for the staff 4. Check MARs for completeness 5. Participate in cholesterol screening 6. Follow other health care professionals at the nursing homes 7. Compound some medications 8. Other small projects related to long term care |
